Things to bring

  • Completed new patient paperwork
  • Government issued ID
  • Insurance card
  • Co-payment or deductible payment

Please note :

1.You are requested to come at least 30 minutes before the first appointment for completing the registration formalities.
2.Missing an appointment without a 24-hour phone call notice will result in a $25 dollar cancellation charge. If you have any further questions, please do not hesitate to contact us at (352) 404 - 7570.
3.We accept self-pay patients without insurance. The rates vary depending on the service being performed.
